Applicant must submit signed request to Chief Immigration Office. Applicant's mailing address and telephone number(s) must be included.
Attach your Marriage Certificate, (original or certified copy)
Attach Birth Certificate for spouse, (original or certified copy)
Attach an original Police Record for spouse
Attach proof of financial support
Letter from bank stating current balance and average balance for past six months;
Job letter; and
Two most recent pay slips
Attach proof of your status i.e. a copy of your birth certificate, bio page of your passport, residence of belonger card, or entry permit stamp and Immigration number (R-___) in your passport.
